FAITH ALYSSA ZURINSKAS,

DOB: 03/24/2010, and

SOMMER JANE ZURINSKAS,

DOB: 10/10/2008,

minor children.

TO: JAMIE MICHAEL ZURINSKAS

YOU ARE HEREBY NOTIFIED that there has been filed in this court a Petition for Adoption and Termination of Parent-Child Relationship with FAITH ALYSSA ZURINSKAS and SOMMER JANE ZURINSKAS. Said petition asks that there be first an adjudication that your consent to adoption of such children is not required by law, and that your parental rights to such children, if any, be terminated.

YOU ARE NOTIFIED that FAITH ALYSSA ZURINSKAS was born on March 24, 2010 at Tacoma General Hospital in Tacoma, Pierce County, Washington and that SOMMER JANE ZURINSKAS was born October 10, 2008 at Tacoma General Hospital in Tacoma, Pierce County, Washington. The natural father of FAITH ALYSSA ZURINSKAS and SOMMER JANE ZURINSKAS is KELLY DEE HAMMER.

YOU ARE FURTHER NOTIFIED that a hearing on the Petition for Termination of Parent-Child Relationship will be held on June 14, 2024 at 9:00 a.m. in Lewis County Superior Court, Lewis County Courthouse, 345 W. Main Street, Chehalis, WA 98532. At such hearing you have the right to be represented by counsel. Counsel will be appointed for you if you are unable to afford counsel and request that counsel be appointed. Your parent-child relationship will be terminated if you fail to respond to this notice within twenty (20) days of service hereof or thirty (30) days if served outside of the state of Washington or if served by publication.

YOU ARE HEREBY FURTHER NOTIFIED that failure to respond to the Petition for Termination of Parent-Child Relationship within 20 days of the service of such Petition, or 30 days if served outside of the state of Washington, or 30 days of the first publication of the notice, are grounds to terminate your parent-child relationship.

FAILURE TO APPEAR AT THE HEARING MAY RESULT IN AN ORDER PERMANENTLY TERMINATING YOUR RIGHTS WITH RESPECT TO THE ABOVE-NAMED CHILD.
